Vaccine Medical Exemption Process for Students
State Center Community College District (SCCCD) is committed to providing a safe and healthy learning and working environment for all students and employees by requiring proof of COVID-19 vaccination as a condition to enter any indoor space on District property, effective October 15, 2021.
Students with a serious health condition who need to request a medical exemption from the COVID-19 vaccination requirement will need to complete a medical exemption request form.
Religious exemption forms will be available soon.
Completed forms must be submitted no later than October 11, 2021. Please allow a minimum of 3-4 days for review. You will be notified of your approval status via your student email account.

- Pick up a form at the Fresno City College Health Services Office

Monday-Thursday 8:00 a.m. - 4:30 p.m.
Friday 8:00 a.m. - 1:00 p.m.
Closed daily for lunch 1:00 p.m. - 1:30 p.m.
3. Email the Health Services Office to request a copy via email healthserivces@fresnocitycollege.edu
The Form:
- Fill in your information at the top of the form
- Give the form to your medical provider to complete
Options to submit a form:
1. Upload the completed form in your Student Health Portal at scccd.studenthealthportal.com
2. Turn in a hard copy at the Health Services Office
3. Email a copy of the completed form to the college:

If you are taking classes from multiple State Center Community College District colleges, you only need to submit your request to one college. Colleges include Fresno City College (including the Career & Technology Center and Training Institute), Reedley College, Clovis Community College, and Madera Community College (including the Oakhurst campus).
